Welcome to the Patient Resources page at Allee Vision Optometry. We’ve made it easy for you to manage your eye care needs with convenient access to essential tools and information. Here, you can quickly fill out patient forms, request an appointment, and review the insurances we accept. Our goal is to make your experience as seamless and efficient as possible, so you can focus on what matters most—your eye health. Patient Forms

For your convenience, you can access and print all your eye exam paperwork ahead of time. Completing these forms at home allows us to prepare for your visit and helps get you checked in smoothly and quickly.

Please note that these forms are not submitted online electronically. Be sure to print out your completed forms and bring them with you to your appointment!

If you’re unable to print them at home, you’re welcome to stop by our office during business hours to pick up a packet, or simply arrive 15–20 minutes before your scheduled appointment to complete them in our lobby.

Existing Patients: If you’ve had any recent changes to your contact information or insurance, please let us know at your next visit so we can update your file and make a fresh copy of your driver’s license and insurance card.

Request an Appointment

Scheduling your eye care appointment has never been easier with our Request an Appointment feature. Simply fill out the online form with your preferred date and time, and our team will confirm your appointment as soon as possible. Whether you’re coming in for a routine eye exam, contact lens fitting, or a specialized consultation, we strive to accommodate your schedule and provide you with the best care. Insurances Accepted

At Allee Vision Optometry, we understand the importance of making eye care accessible and affordable. That’s why we accept a wide range of insurance plans to help cover your vision services. Whether you have private insurance, vision plans, or participate in flexible spending accounts (FSAs) or health savings accounts (HSAs), we work with many providers to ensure you receive the care you need. To find out if your insurance is accepted, please contact our office for more information. We’re here to assist you in maximizing your benefits and making your eye care experience seamless.

Cancellation/No show policy

Should you need to cancel or change your appointment, we do require a 48 hour notice if at all possible. If you fail to give a notice of a minimum of 24 hours prior to your appointment it will be considered a “No Show”. If there are three (3) cancellations (less than 24 hours) or “no shows” the patient may be dismissed from Allee Vision Optometry. There is a no show fee of $40 for each no show.
